Rudder pedals can be a great thing for MWO, but this item is way beyond that. This is the equivalent to a D-Pad for your foot. You can also use multiples of these at one time. 4 buttons on this you can assign as you like which is something a set of rudder pedals can't do.

A bit expensive for a d-pad.

Pro Tip: A gamepad on the floor makes an excellent VOIP Push To Talk button.

Best to use a generic (NOT XBOX) pad, because xbox pads can be recognized by games (eg MWO) and be mapped to things with no way to disable it.

Use the Analog sticks for your buttons - they click in.

I use the left one for lance whisper and the right one for general chat.
