If you're having trouble with jams try delaying the second shot as late as possible in the cooldown cycle.

The "hold to fire" command was given as a treatment to the pre-first-shot-jam. Since it has become quite a bit rare.
What happens is with computers set to make 'double tap' registration easier might accidentally assume that what was intended as a single click, because pressure was applied, let off and reapplied ever so briefly, was a double. If you double tap quick enough before the first shot fires, you instantly jam without a single shot being fired.
Just like, if you don't jam but do it... you can get 3 shots off in a single cooldown.

Looks like you pulled the trigger just before the cooldown, which means that you basically got the double tap to happen at the last possible second... then Immediately afterwards, you fired the first shot from the next cycle.
To do some ascii art:
Say "i" represents the initial shot, and "!" represents the double tap.
firing without double tap would look like this:
i-------i-------i-------i-------
Adding in the double tap, assuming you do it as early as possible, would look like this:
i---!---i---!---i---!---i---!---
However, you can delay the double tap, as long as you want, as long as it is before the cool down cycle completes, like so:
i------!i------!i------!i------!
This creates super-rapid succession shots. In fact, I used to try and do this ON PURPOSE (though with varying degrees of success) because it can basically turn two UAC5 shots into an AC10 shot.
From what I can tell, your "triple shot" was actually a combination of one very late double-tap, and a 2nd early as possible double tap, like so:
i------!i---!---
............^ "triple shot"
i---!---
..^ normal double shot
I don't remember exactly what the double tap window is... lets say its 0.7 seconds.
It is possible to fire 3 shots in ~0.71 seconds, because one is a late double tap, from the previous cycle.

and btw: why is everybody calling for a video proving it? just try it out by yourself. this bug even happens on the testing grounds.

I never once had it jam on the first shot. It has always fired at least 1 shot for me, before jamming. For me, "jamming on the first shot" implies that it does not fire at all. I have never seen this, in hundreds of games using the UAC5.
I have had it jam after the first shot due to an accidental double tap. (The game used to be more sensitive to this with the old mechanic)
I have always known about the alleged "triple shot" and I never considered it a bug.

well then you're lucky.
i've got it quite often that it jams without even getting a round out of it.... and i always start cursing when this happens.
i made a video about it a few months ago and send it to the support, the GM who answered confirmed that it's a bug and they know and working on it. Unfortunately i deleted that video after the email conversation.

the first engagement i had was a raven, nothing fired before and my UAC5 jams right after the first button push.
Ok it gets a round out, you're right, but then it instantly jams, this is also a bug and not "working as intended"
since i didn't tried to double tab. Even if you hold down your firing button it shouldn't jam, it should just fire like
a normal AC5.
