I know before there is even a reply to this thread, that others are going to have different opinions, and that Community Warfare is most likely going to top this list, if not for "embarrassing" but for "BIGGER", but I wanted to dredge up their C3 comms system.
Does anyone even use this? I don't recall ever seeing a player message, "Hey hop on C3 with us" or get an invite over the past 6 months.
My club and our players use TeamSpeak 3, we can invite people to join us, and even though it's not "one click easy" it's a lot simpler than using C3. I would still like a built in method that allows us to connect with pubbers though.
One a priority list of things to address with the game I know that voice comms is also way, WAY, down most players lists, and for that reason it's unlikely that the Devs are putting any resources towards it at this time.
Why isn't it possible to implement a Battlefield 2 type voice comm system, that's invisible, automatic, and works well? I'd imagine that just licensing a system like that to other companys would be a lucrative side business, but I guess that if C3 is what they thought was the best option, the question answers itself.

C3; Mwos Biggest/most Embarrassing Failure?

It would be fairly high on my list actually. I like puplic games. I hate the silence. Some coordination can arise anyway, with some comon sense, a bit of instinct and cautios use of text chat.. but it´s hard. Why a game with a decisive focus on teamplay has no ingame voice chat eludes me everytime I play.

Well, since PGI didn't create it, I don't see how it's their failure.
But if you're talking about intregration, this is from the ATD 36:
zhajin: What is the status of C3 and voice comms? This is a key communication feature in any modern, team based FPS. And continues to give a big advantage to groups over pugs.
A: Post launch.
That tells me that it's pretty low priority. Just because it was put on the back-burner as a solution to something that isn't really a problem, doesn't make it a failure, and you're just speaking in hyperbole.
Besides, it seems that most Houses,Merc Corps, whatever seem to already be using TS3/Vent/mumble services, many of these have been using the same ones for longer than MW:O has been a 'thing'. Plus you have NGNG, Comstar NA, etc available so the problem has resolved itself. I know that this doesn't help the PUG... but the tools are out there if you want to use them.
I'm all for integration, but it seems like a 'nice to have' over a requirement. Also, seeing the text that's spewed in chat when I'm pugging, I sure as hell wouldn't want them in my ear, or coming over my speakers.

This I don't even consider it a failure because they didn't even do anything with it. If you want third party voice you're always going to go ts3 or another.
C3 needs to be part of the client.
