Recently I've been pondering making a pen and paper RPG inspired by the Dark Souls universe.

Story is set in a medieval like time period where the last bastion of humanity lives within a walled city ruled by a dictator king. Outside the city walls is a nearly inhospitable literal Hell-On-Earth. It is split into 7 demonic kingdoms, each one ruled by a once-human-but-now-horribly-mutated-and-disfigured Royal Demon/Demoness who represents a cardinal sin, and inhabited by bloodthirsty nightmare fuel and mini-bosses for the player/s to fight.

The end goal of each campaign is to defeat all 7 of the Royal Demons and then defeat the "final boss", with each campaign, including the monsters, locations, and forms and abilities of the bosses, all being determined by the DM so playing through it goes differently each time. Doing so opens the gates of hell and saves humanity; but only for 100 years. After which, the new generation is cast back into Hell and the cycle repeats.

I've never played a pen and paper RPG before but I'm just looking at mechanics and things and tossing ideas around. Damn, I need to play DnD one of these days.
