I have been having the mouse cursor show up in game more and more frequently since the last patch and it has got me into some bad situations (my mech goes idle while "tabbed out" and the enemy takes some nice free-bee shots). I have read up on how to "prevent" this by making sure not to tab out of the game while loading a match and it prevents it while starting a game for the most part BUT it still happens from time to time.
I have, however, noticed what brings it up in game and can repeat it for myself 100% of the time either in full-windowed or full-screen. I have windows 8 and because of micro$ofts' "start menu" implementation in the bottom left of the screen (mouse gestures as it is called) any time i swing my mech left and down while pressing the fire key in the motion, it briefly brings up the start menu (metro) and quickly returns to the game but now with the MWO cursor. I run multiple monitors and now that i have the cursor floating around, i inevitably click out of the game and get tabbed out, usually in heated engagements.
I read the new UI is on its way but i ask this gets looked into sooner than later because it is VERY annoying and seems to have been brought on by the latest patch.

A company called StarDock makes a mod for Windows 8 called Start8, which provides the user an option to turn off Metro, and reimplements a pre-8 Start menu. I'm not sure it will solve your problem, but it's probably worth a try. It's free to try, and $5 to buy.
