To me the major issue with 12s is lack of teams or Pugs doing them.
This is caused by 2 issues.
1. Difficulty setting up a 12 man. 3x as many people to hit ready with no real social aspect in the game, makes it fairly frustrating for a "casual" 12 man.
2. Rewards. Playing 12s is harder, and does not offer increased rewards for doing it. Matches last longer, you die more often, yet you are not given higher rewards.

Kageru is absolutely right with his analysis on skill disparity, but would point out one other thing I've been learning as I do more 12 man, 4 man and sync drops: Corps Philosophy matters a lot.
Been hearing from other pilots who've been in different groups that the personality of the merc corps out there varies greatly. Some are ultra competitive where they monitor your KDR, Dmg and other metrics in game and have minimum standards that you must meet. They take it all very seriously and with great intensity and dedication.
Other groups are like Church League Softball with a kegger of beer at second base. They aren't out to be the best that can be. They're out there to have fun and enjoy the benefits of working together as a team but they don't focus as hard on performance.
The "Church Leaguers" aren't automatically rolled by the "Ultra Competitors", So don't consider that to be the reason of disparity. At the same time, don't forget those 12 mans are always tough.

I think that is has been said many times in this thread that 12 mans are more competitive. I have to fully agree with that statement. Players that dive into 12 mans and thinks it will be the same as a pug match have a very large wake-up call waiting for them. As it is in many games, teamwork can be a huge advantage to a team. Teams that play together for a long time are going to be much better than a random assortment of people that have maybe never played with some of the others on their team.
PUG matches exist for the casual player. PUG matches are for the player that only has time for a couple matches a night, or maybe doesn't have a set schedule to play and can only get in games when they can. I just wish there was a way to quickly send friend requests at the end of the match to other players that you worked well with.

First of all, I'd like to say that I completely agree with your statements here. However, I believe you're somewhat missing the point of the OP.
From my understanding, the concern is that the 12-mans will completely run Community Warfare. The entire IS will be controlled by a few of the top 12-man Teams, and the rest of the players will have little or no impact on the events that occur.

Well if they make it so you have to chose a region where your unit will be fighting in the IS and then actually make it take a few days real time to travel to a new area then the big 12 mans might all congregate to an area close to Terra leaving the more distant reaches to the lesser 12mans and soloists.
Don't think I'm saying your account is unuseable for a few days but that the 12 man team would have to submit a ledger with their team make up and those mechs will be locked out for X number of days. The players could still drop in other mechs, just not their prized Babies. Once the time limit has elapsed the mechs become useable again.
If they don't do something like this then CW will quickly become something that most Soloists and many "Church leagers" will want nothing to do with and unfortunately as CW will be the core of the game it will mean a lot of people moving on to a new game. Already a few voices out there are asking about new titles coming out in the next few years, if anyone is interested in doing something there. PGI needs to get this one right.
