I have already submit a support letter regarding but Im curious have anyone else encounter this icon.

See the little grey upside-down stick man icon at the top left of the screen.
Whenever it show up, Im sure to encounter the black screen bug for that match.

Yup I've seen this too; I can't help but wonder if it's the Dev's way of trying to identify a possible problem server in rotation through leaving a visual clue. I've done that in the past while trying to debug problems in a Test environment.

I have done something similar for debugging too, have the code spit out a message at each interval.
Hopefully this screenshot will help the dev quickly identify the cause.
