Probably either by securing an almost capped point from recapture by the enemy or an almost (un)capped from hostile takeover. Would make the most sense to me.

I'm not sure how to get it properly.

could be. I've only seen it 2 or 3 times...and can't remember specifics of them....

Or maybe killing someone while you are capping a point?
