Always run the patcher repair tool after a patch.

havent had a drop in fps in as long as i can remember, but the detail you gave us should allow us to help u immensely.... btw hardware section is a good spot for solid tips on performance

Did you try rebooting. I had an issue where the patcher was using up my cpu and making my computer hard to use so I changed the affinity. It had the undesired side effect of limiting the cpu of the game itself even after I set it back.

I found that for some reason MSAA was enabled and murdered my framerate.

New map kills me frame rate at the start of every match for a few seconds, and then periodically drops to from 35 to ~22fps during gameplay, especially if using heat vision

FPS hasn't dropped for me in awhile. There was a patch many months ago that upped my framerate, but the next patch knocked it back down to original levels.
I would contact support about this if you havent' already, as that is a huge performance hit.
