Has it ever happened? Has someone somewhere managed to win an attack by killing all 48 of the defenders?
I'm thinking it is impossible, I might be wrong.

Even if you kill 48 mechs you still have to destroy Omega.

I've been in defenses where we lost all 48 mechs, but yes, the attackers still need to pop the gun generator.
Generally it happens when the enemy team has comms and yours doesn't, and even more so if they bring a wave or two of specialized sniper build that they know how to shoot with.

My group did it with 8 minutes left to destroy Omega.

I've done it, but it was an organized team against pugs and even then it can be a gamble.

I have. You have to play the map though, and the team has to understand that once past the gates the primary focus is to gain a kill lead over the enemy. Once the enemy team is off balance and stuck waiting to drop, you focus on keeping that lead until it is impossible for them to close the gap.

I saw it happen (to me) once. It was a fairly decent fight at the start, fairly even losses for the first wave, then the second wave just owned us and it was a single-file march to death after that as nobody on my team would wait at the dropship - though the enemy would have likely just blew Omega while we waited, had we chose to do so. I think the end score was like 18 / 48 or thereabouts, and they easily got the gen once we were dispatched
