Anyone know how to test for this? I can check ping and do a trace route, but any problems I see there can't account for the terrible hit reg that plagues me

I have perform well at ping 350. 2 kills something about 600dmg. Unfortunately I lose screenshot after game folders was moving.
Packet loss is more valuable issue. But I don't know built-in tests.

You could always check with Wireshark, although if you are not familiar with network technologies, it might be hard to read. Basically, if you see a lot of retransmissions in MWO's traffic, you are suffering from packet loss.
350 ms latency is very high. I doubt HSR can give you a constant result here, it is bound to be bad sometimes. Without HSR, I guess you would have to lead your shots by two mech widths at medium engagement range.
