I am running a 144hz monitor but whenever I turn on V-sync it gets capped to 60FPS.
Is there a way to customize V-sync settings so I can throttle my limit to 144fps?
Otherwise my GPU will run at 100% at somewhere around 220 FPS and get really hot.

Turn off vsync in the game, turn it on in your driver settings (be it Catalyst Control Center or nVidia's control panel) - just make a profile for MWO. Your driver settings should allow you to pick an appropriate framerate.

What pc specs to have 220fps in this game? With what video settings?

You can limit your fps in the config file. Turn off vsync and try searching for the parameters in the forum; couldn't quite remember.

You could try in user.cfg with
sys_MaxFPS = 200;

You can easily get there while in the dropship before a River City match starts. I've gone as high as 280fps. Drops below 200fps once dropped. i7@4 GHz with GTX950.

At Max settings or low?
I'm running 4790k and GTX970 and sit at around 60 with everything on ultra

Custom settings, with some at low. I had to bump some of them up to curb the fps when dropping in. I'll check the exact settings later. Too much visual noise for me to run high settings. The eye candy just gets in the way, in my opinion.
