When using multiple monitors, mouse cursor randomly doesn't lock to that monitor which is used to play.
If you have other programs open in other monitors, cursor jumps to those and game can all of a sudden disappear to backround while toughest fighting. Guess what the **** happens then?
Been issue over a year now.

Since I use 2 Monitors I also have sometimes the same issue, if working on the other not game-Monitor before the mech drops.
On searching for a match I move the mouse out of the Game Monitor to my main Monitor doing browsing or often Excel-calculations and formelwritings and then the game snaps the Mouse to ingame while connecting - that often disturbs the writing and then I have to move it out again with Alt-Tab only to loose my mouse again in the dropwait...

That only happens with dual Monitor never with one Monitor, where I always Alt-Tab out of the game (full window).
Also the game doesn't remeber on which Monitor I started it - It will always start on the Main Monitor although I moved it to the second monitor (the patcher and the game intro).
the only thing that helps is to make my second Monitor my main and after starting the game switch my original Main Monitor back to the Main

Here's a tip I use to avoid that exact situation:
During the countdown screen prior to starting, push the <ESC> key.
Then use your mouse to click the <Resume Game> button.
Don't do anything else. Your mouse is in the center of the screen. Leave it there until you drop.
Since I've been doing this, I have never had my mouse lose focus.
By starting in the center of the screen, you won't move your mouse enough to go "off" the screen. If it does, your mouse sensitivity is probably too high.
