This has happened on two occasions now. Armor is in the yellow and internals are red or destroyed. Is this working as intended?

Welcome to the forums.
It appears to be death by overheating. You're getting too hot.
Are you using standard heatsinks or doubles? Are you using "Override"?
Here is a way to see damage being received by overheating.
Mind you this is barely incurring the wrath of heat... while using standard heatsinks.

Actually, FAR better way to see it.
This was when the overheat penalties were really soft.
36 PPCs fired in 18 seconds for 288 heat (8 heat per ppc, 3 seconds recycle time, macro to make it look cool).

So yes, working as intended. Three ways to cause internal damage without destroying armor:
1- overheat and shut down: Causes damage to CT structure only
2- overheat with override: Causes damage to structure of random components
3- MASC use beyond threshhold: Causes damage to structure of both legs
From the OP's image, it appears the mech was overheated while override was engaged. Fun!

Yeah that looks like overheat damage to me. Did you overheat-shutdown in the middle of a lava flow? That would cause extra heat damage because your mech is effectively still gaining heat above 100%.

In Closed Beta, the Catapult 'ears' were vulnerable to 'through armour crits' with the doors open (lost the launcher twice to LRM5 hits). It has since been fixed.
