This has been happening more often since a patch or two ago. If you Alt-Tab or whatever back into the game as the match begins, the game will randomly lose track of the mouse when you press a button somewhere in the MIDDLE of the match. It used to be that if you simply clicked the mouse button before combat began, you may have the first click bring up a background window or click on the desktop, but the second click would properly link the mouse to the game and you'd be fine.
Every since this bug started, you have no idea when the mouse will desync, resulting in basically nothing happening when you click the mouse, often getting you killed as you try to re-link the mouse to the game.

This *sounds* like the issue which returned a couple of patches back where, if you have two screens and alt-tab out of the game in full window mode (maybe in other modes too, not sure), the mouse isn't fully captured when you first alt tab in and so, when you twist sufficiently far to one side that your mouse cursor would appear on the other screen if you were just on the desktop, it does just that even in game. If you then press a mouse button, focus switches to the desktop/whatever window you clicked on, and you lose control of your mech.
With two screens, what I do is to drag the mouse off to one side after match launch. If the cursor appears on my 2nd screen, I alt-tab twice ("out" of game, then back in) and that fixes the problem. Might work for you too until it gets re-fixed.

(nods) basically there are two mouse cursors, one in the game and one behind the game. Left click on a desktop icon unknowingly and it starts that program and the game loses focus. Right click for weapon group 2 and time it just right click on the desktop options, steals focus...

Can confirm. This was an issue that they fixed at some point either last year or late 2014, but broke again recently (late Feb.-March). It's pretty annoying, but I've just started alt-tab'ing back sooner and doing a manual check for the mouse focus so I can fix it before we're all out of the DZ.
