I just got a logitech G402 and can not assign weapons groups to any of the buttons other than mouse 1 and 2 and the scroll wheel. Anyone else had this problem. I used to use a G500 and had no issues.
Any thoughts?

gaming mouse tend to require an extra software install to customize beyond 2 buttons.
example: this is the software that come with my Armageddon mouse.

a quick look at logitech support page show they too have a control software for gaming peripheral.

I use a G502 and just assign the weapon groups to the buttons by remapping within the Logitech software, for instance mouse button three (press scroll bar) to 'R.' So that's another approach to try.

You need the Logitech gaming software, in fact, my G13 gamepad does not work without it. With it you can reset buttons on your mouse. I have the same mouse BTWay.

Did you try the mouse in game before trying to assign weapon groups, be it in the testing grounds, etc? If not possible first error.
The #4/#5 mouse buttons are setup for the BACK/FORWARD (as in IE browser function) and MWO defaults to them to fire group 3/4. Others who had issues had to set those buttons to generic functions.

Also, what changes have you made to the in-game controls?

.....................................Key Map.....Alt Key Map
Fire Weapon Group 1.......1..........Mouse 1
Fire Weapon Group 2.......2..........Mouse 2
Fire Weapon Group 3.......3..........Mouse 4 (3rdpty mouse software, tis listed as Back (back a browser page))
Fire Weapon Group 4.......4...........Mouse 5 (""" this is listed as Forward (forward a browser page))
Mouse 3 is the scroll wheel (zoom stuff)
If you changed anything on the 2nd column (Key Map), change it back to match the above. Use the Number keys above the letters, not the keypad itself.

Yep, exaclty as above Tarl Cabot said, I had similiar problem with Microsoft mouse, I disliked the forward back browser functions of mouse button 4 and 5, disabled them from the Microsoft mouse center that was installed with the mouse, and thus they can't be assigned in MWO. Reassigning them back will allow them to be identified in MWO.
