Honestly I wasn't able to use the MASC before getting a proper gaming mouse. So a lot of gaming mice has the 4th and 5th mouse buttons right above where you put your thumb, and so I push my thumb up to activate MASC. Before my thumb was mostly used to grip the mouse. With the 2 above buttons you can grip and MASC at the same time.

I've got one of those Razer gaming keypads for my left hand, where there's a set of keys positioned specifically for the thumb. I got it to keep some of the wear and tear off of the left side of my keyboard, but when PIG added MASC, the layout turned out to be handy.

It's got the space bar positioned below, and the equivalent of left-alt positioned above the thumb's neutral position. It makes for an easy reach, and means I haven't had to rig the MASC to my mouse.

I get that it's not a solution for everyone, but that keypad's been a nice one for me over the last couple years.

I use a Razer Nostromo and have also mapped my MASC to the spacebar button. It works brilliantly like that.
