Not sure if this is OPs experience.
But I would count it as system issues rather than hacks.

oceanic likes to do this thing where it disconnects everyone and wont let most of the people back in. the ones that do get back in get to farm all the dcs.

I still place bets on at least 90% of the people with "Death Star" only having it because of that server error. Damn vultures.

PGI reneged on their promise to roll back the title for people who got it during those server bug periods.

i hate that mechs can just phase through eachother. i know one game i had a light walk through me pull a 180 and blow out my engine from the rear.
i found that one time they attempted to put collisions back in, and light mechs would just get stomped to death. that was funny as hell, i know i tked a few of them who were walking abit too close to my assault mech. that was a fun few days before it got patched out.

They need to start prosecuting these hack makers and possibly users until it stops. It's one thing to make them for single player stuff that has always been made. Which can be fun or even add to a game. But when you get to online play and peoples time and money is being affected there is no excuse for this.
In theory, this type of hack is impossible with MWO ... which is why I am dubious. Unless PGI has disabled server side sanity checks ... (which isn't impossible but would be incredibly stupid) ... any mech should not be able to "teleport" more than a few meters - the distance it could move between position updates assuming some dropped packets.
Similarly, fire is resolved server side, so a mech can't teleport behind an enemy ... fire ... and teleport away. The game doesn't work that way unless PGI has broken the server and is accepting client side input as fact.
One of the advantages of MWO is that it is supposed to be server authoritative which means that the server should be able to flag hacked input from the client and then both ignore the input AND ban the client.
Anyway, I'd like to see some video of this teleportation (other than simple rubber banding) especially with them teleporting several hundred meters behind opponents, shooting and teleporting away since it should not be possible in MWO.

This is in Tier 5 BTW.
I payed for these mechs. This crap has no business going on at this frequency. It's literally every game. And you cannot use Masc to try to get behind mechs when they can all teleport and a constant frequency. This stuff needs to be stopped and it needs to be stopped now!
And they are not mechs touching each other. It is always mechs warping to the most advantageous position at the most advantageous range. Side or back and right where they need to be.
If you are seeing it every match, video it, upload the video and link to these forums and report it. Personally I have not seen it but would be interested in seeing what is going on but evidence is critical.

Things that DO happen are aim botting and lag switching and both are quite obvious. Aim botting can be observed when specing a team mate, watch for an unnatural 'snap to target', especially at very long ranges, where the same component is hit repeatedly without any noticeable damage spread. Lag switching is noticeable when approaching an enemy mech where you will experience an extreme lag spike, often as long as 3 seconds and very repeatable, you can also hold TAB and watch as multiple players ping goes absolutely nuts.
These are warning signs to watch for as some players are just disgustingly good shots and sometimes the game servers are just spazzing out due to the computer hampster having a seizure over the cries of "KDK3 OP PLZ NERF". Just remember there is a report player function and PGI most certainly pays attention to these reports.

Imo, you have connection issues.
packet loss?
Anyway, It doesn't matter if you have good ping, good lag, etc. BUT your provider buffer send/receives large package datas per second, then ...you must phone your provider and shout at them.
use this test (not stupid speed testers u can easily find in google):
