I Killed A Team Mate Unintentionally

Yesterday, I killed a team mate unintentionally. I was equipped with 2 LRM 20's and a pair of med ER's.
In the quick play, I noticed a team mate fighting a larger mech on his own and targeted the enemy mech with R and proceeded to fire my LRM's. At first, my team mate was hidden behind the enemy mech from my view point and continued to fire my LRM's.
As the fight went on, they both turned 90 degrees and I had a broadside view which showed me they had some respectable space between them, so continued to fire my LRM's. Next thing I see they turned once again where I could not see my team mate and while this was going on, I was still firing my LRM's. Next thing I see, the enemy dies and a split second later, my team mate as well. My team mate was credited killing the enemy mech, but I was identified as killing my team mate! WTH??

From the beginning of this fight, I made sure I had the enemy mech targeted while I was firing the LRMs. I never heard or seen anything like this before and well, like you all, hated it when I ended up killing my team mate unintentionally. I tried to contact the team mate, but he went off at the same time I was killed.
I waited out the battle and tried to screen print the battle results, but all got was a blank screen when I opened it up in my Photo editor.

My question is, how often does this happen? Is there a way to contest this and if so, is it worth doing so? Or should I just say, “Crap Happens”; and let it go?

Opinions?
Thanks all!

Anyways, crap happens, the LRMs probably passed through the corpse of the enemy and smacked your ally. As long as you aren't TK'ing on purpose, it's not a big deal.

Accidents happen.
Just remember;
If your not willing to shell your own position, your not willing to win.

The only true offence here is him playing an LRM boat :^)

Just last night I TK'd a fresh team-mate. Now that's hard to forgive.
I was in a HBK-IIC-B (4xASRM6 + 2MPL) and he was in a SHD-2D2. We were flanking wide on Polar, I was on his 4 as we stalked a Marauder IIC. We got a firing angle, and I poptarted (I run 4JJ in my HBK-IICs) to alpha over his shoulders.
The shot would've been perfect--if we'd have been on flat land. But he either went up a hill or poptarted himself, and my alpha went straight into his ST, killing him instantly.
Worst part is we stomped the team. Dude was our only casualty.

I accept your apology, as this happens. To bad the penalty timer does not.....it hates everyone with equal satisfaction.
