Very first thing. Reboot computer...
Then about VOIP, are you speaking about the in-game VOIP or Teamspeak/etc?
You may have had some OS or GPU updates since by default driver updates are enabled by default for Windows 10. GPU update may have changed the default Sound Playback selection. If you are using a 3rd party sound program that could affect how MWO is interacting with it.
Right click on the sound icon first then playback devices. Double check that the correct Audio is set as Default Device. Then log back into MWO, settings and audio. Make sure nothing is changed or forced there.

try this, right click the speaker in the taskbar select playback devices speakers then the communications tab then select do nothing.
the setting i am talking about should be the same no matter the playback device ,it turns off other sounds when a communication program is being used.

I'm willing to bet you have a different default audio output set within MWO, or its something to do with it anyways. I routinely switch my audio device from headphones to speakers (through HDMI) and if I have it through speakers and switch through headphones after the game has already started, the original audio output will still be in MWO (ie. sound through speakers, not headphones) until I quit and restart the game. Interestingly though, VOIP switches to the audio output that I want (so I only hear VOIP through headphones and no game sound).

This issue is still on-going to this day. I have also ran through my entire build, drivers, outputs, troubleshooting, and all this audio issue is still persistent. Its been months of troubleshooting and I'm honestly done trying anymore. Sad when I really wanted to check out the updates.

Reverse your system back in 1 week or something.

Two posters in seven years doesn't very strongly indicate a widespread issue - there's almost certainly some setting on your computer that you can change to fix this.

Typically its the output section in the audio tab that gets changed from your actual output device to something different. No explanation on how or why it happens but changing that back to whatever your pc is using for its audio fixes it.

Though you may have been trying to fix it for months, ya only posted your issue this week. Adding this to this 7-year old thread additional steps. Gotta love MWO search engine.
When uninstalling the game, it does not delete the config files in your Saved Games folder.
Have one question (this was my last advise but moved it up to the top), are you using a headset or just a mic, and is said devices plugged into the front panel? Start > Search > Realtek Audio Console. Device Advanced Settings and check the Separate all inputs jacks as independent input devices.

- For your system, System > Sound > More Sound Settings (near bottom of page).
- Another box will pop up. Playback, click on the device that should be playing your sounds (it should have a green check mark next to it). In this same tab, you can disable the other devices, especially those attached to a monitor or Steam.
- Select Properties for another box. Advanced Tab and uncheck the Allow Applications to take exclusive control of this device.
- In the System > Sound> Volume mixer
- Step one done
- Do not be logged into the game. Drill down C:\Users\Computer name\Saved Games\MechWarrior Online.
- Click on Profiles, the toon's name and rename or delete Attributes.xml file. This will reset your game settings.
- If/when any preferred device is changed in the Game's sound settings, you will need to log completely then back in for it to take effect.
