99% of Her freezes happen in that moment. Map and game mode not relevant..

99% it is conflict with sound driver. Cryengine is known to have such issues. Update sound drivers, and repair game installation (look in the game launcher for repair option).
Edit: Another option could be Skype or some other sound messaging program hanging in the memory. Unloading any such program is very important Pre for smooth gameplay.

What's hilarious for me is that now, even in fullscreen MWO will occasionally minimize.

Usually in a match, too. I think when they chose Flash for game UIs, they chose (again) poorly.

Yeah, this only happens rarely for me, but when it does it's 2-3 times in the span of 30 seconds.

Getting back in game to find out you were in just the wrong spot to get instagibbed is never fun.

Defragging a HDD is basically lining up all the information so it can be read faster on the spinning disk. In an SSD, there are no moving parts, the way that data is addressed is entirely different. However, the cells in an SSD that the data is written to can only handle so many write operations. A defrag will rewrite pretty much the entire drive, lowering the lifetime of the SSD for no performance gain.

I also feel like the game doesn't like alt-tab alot, and if you alt-tab at critical junctures eg when the loading sign isn't visually spinning, or when the game is transitioning between the UI and the 3D engine when you're alt-tab'ed, things can go haywire.

That kinda sounds like a video driver crash & reset..
Disable any video overlay options you may have enabled, ie: the Steam Overlay

Also if you have an NVidia card, turn off ShadowReplay for recording, the current driver state is buggy and an insane disk hog (NVENC is a crappy codec) that can't write to memory without a ramdisk app. If you want to record it's safer to go with a software option, or if you run Win10, use the now-new built-in GameDVR as part of the XBox shoe-horn with Windows 10. If you have Win10 and installed the recent Fall Creators Update, hit Windows+G for options. It does 10mins instant replay or 4 hours recording 30/60fps and also does streaming, and yes for instant replay it writes to memory first, 1.6GB for 10 minutes 1080p@30fps, 128kbps audio .

I use the MWO portal, not Steam. Only things I have running are PC meter (my CPU used to get hot, fixed now, should uninstall, too lazy), AVG & the Sound Blaster control panel (so I can swap between headphones & speakers). All the Nvidia gumf is disabled. MWO hates windowed mode on my PC, so fullscreen only.

It's not a regular enough problem to bother me & since the AFK penalty changes, I've only received a penalty for DC'ing & reconnecting once.
