What are people's ideal methods for hosting images view-able in the forums?
I mean free, no gimmick, it's 2018, no extra complications or headache solutions?

I prefer postimage, no signing up and no loops to jump through to get a postable link.

What is the code involved to link it into the forums? I think in the past it was [/img], or some such.

I use imgbb

Fast, easy, no bull.. and no photobucketing (was free, but now I make you pay or withhold your own images, haha!)

I use imgur too.

I would however not expect any free service like that, to be able to run as free service indefinetly. Some might be able to, but many have failed. In 2018 people shouldn't be surprised by that.
