One labour-cheap way to do this quick would be to simply allow people to upload custom images to use for their pilot face, of given dimensions.

I voted yes, but it was a realistic yes. Get the game out, then give us the ability to make an avatar. Even if it's just a "mugshot" in one corner of our pilot skill tree.
All the other stuff can come down the road. Hayashi's suggestion above is a good one. Set a threshold of X number of reports warrant a actual review of the avatar by CS would keep the costs down as other wise it would either be useless to report or cost way too much.

I like this, dear god do I love this. Unfortunately, this sound more like something that would come out of a big budget MMO like Star Wars The Old Republic. As much as I love the idea of this being in the game, the more realistic side of me knows that MWO simply doesn't have the budget to include it.

I believe that this will happen eventually. However, until a time when individual pilot avatars are up and walking, perhaps a simple avatar for a players profile will suffice. Think of it as a precursor.

From start use placeholder, some default 3D human model that sit in every cockpit.
its not just asthetic, taking out pilot is brutal, but viable strategy

I think while out of the mech there should be a chat window ala EvE ...not WoW or WoT ... The Eve window allows you to have real conversations. It informs you clearly of who is online that you know. It automatically groups you and informs you of who is online in special in game affiliations. e.g. your corp or alliance or faction.
The wow / WoT model does not encourage interaction. as the information for who is online is separate from the location you actually chat.
