It used to be even worse; when they dropped the CryEngine 3.4 update, all the 'Mech glow effects intensity got like quadrupled or something, which of course made the HDR/bloom even worse. The glow has been decreased back to proper levels by now, but the HDR/bloom issue still shows up some.

It's pretty much anything up close and really bight. Atlas torso, missle volleys, sustained AC fire, whatever causes sustained or flashing bright source basically darkens the entire rest of the screen. Forest Colony is the worst offender.

Does it also happen a lot on Forest Colony Snow? I do not recall noticing it there, but I do tend to let those two merge together frequently.

It makes aiming very hard and is a bonus to mechs with SSRMs on top of the 100% hit chance.

Usually them hitting me doesn't do it at all, but someone firing lrms right next to my cockpit will cause it pretty much 100% of the time.
I'm sure there are other factors as well, but it seems forest colony is the worst offender of it for some reason.

forest snow is whole lot brighter so while it does happen there its detriment to your visions is not as severe, however in the right conditions it can still be disrupting.
