Point A is quite true; there is also the issue of bots, or at least macros. Point B is wildly uncommon (I'd almost say whale and bowl of petunias falling from orbit levels of uncommon, bit it could happen) however it is still valid.

Problem is that a lot of legitimate deaths take place in the first five minutes, your time is too long. And even if it were a time frame where no one actually trying could possibly die, a sufficiently developed macro or bot could subvert it.
To fix the problem, you need to remove the incentive for AFK/suicide abuse (not all AFKs are abuse, go look up the known issues board). The incentive is gaining money and XP for little or no effort - which the recent economy changes started killing. Now on a good match my "I was here" bonus is a fourth or less of my check, rather than half or more. This probably needs to be taken further, and include xp gains, but its being handled.
