I've been playing for about a month now and have still have major issues with aiming. Trying to move and shoot has useless unless maybe it's stationary and I walk directly towards it. Turning/torso twisting to hit someone moving around me never works it seems.
I have upgraded from using my wireless trackball to a M65 vengeance mouse that got very high reviews. It did help but no as much as I hoped, mainly it has more buttons allowing me to assign more to the mouse but aiming still sucks. I have a sniper button and 3 adjustable settings to play with for DPI.
Right now I use a 2500 DPI and snipe with a 400 DPI, I'm not sure however if my DPI is the issue or if it's still me. Are there other settings I should be adjusting or turning off on the computer or mouse? If I'm able to adjust the mouse DPI should I keep the in game mouse sensitivity setting at the 1.0 default?

The 1.0 in game sensitivity is most likely the problem.
I would suggest you set it to 0.1 (minimum) along with smooth and acceleration to the minimum too..
Then try playing and adjust your mouse DPI up as you feel comfortable.
These are my settings:
- In game Sensitivity 0.3
- In game Smooth 0.1
- In game Acceleration 0.3
- Mouse DPI 4000
By the way when I started, everything was set to minimum with DPI @1000.
It took me quite a long time to steady aim until I could move the settings up.

I don't touch the games' mouse settings, but I do tinker with the mouse drivers'.
Ballistic Acceleration et al. is personal preference, but I've heard other guys talk about dropping all the way down to 300 DPI …

What does the in game setting for acceleration and smoothing do that everyone wants them off or turned down? I'm a bit of layman when it comes to mouse mechanics, hell I just learned what DPI meant.

.... Just read a few of the articles listed and all I can say is wow. Who would've thought moving a mouse could be made so complicated. Heck and even stuff like my screen size and refresh rate factor in to! Ignorance is bliss.

Aim and mouse settings = can of worms.
For a start its a highly personal thing, preference is everything.
Then you need to worry about your hardware and software settings.
There's in game factors to consider too. (E.G. armlock on has slower torso twist than armlock off)
About the only thing that is universal (and this is debatable) is that for MWO low(er) sensitivities are best compared to other first person games ive played (eg MWO i use 350 dpi, Day of defeat 600 dpi)
really the only way to truly hone it in is trial and error.
I can list you my settings but that would not help you, thats what feels right for my mouse and my preference.
As long as I can sweep my mouse once and turn my view to full lock, sweep again centre, sweep again opposite lock I have the configuration right for me. Others want a single sweep from full to opposite lock, find that out for yourself.
Aim ultimately boils down to literally point and click. Sorry to sound condescending, but that ultimately covers all there is to aim. Getting your settings to the point where point and click is natural is the hard part.

I've noticed lately that precision mouse hotkeys/buttons do not work when in game, but work just fine in the lobby and mechlab. This is not a driver issue, it is my assumption PGI has ******** the function, at least for my R.A.T. 9

Anyone who plays mouse aim games with mouse acceleration on is seriously hampering their potential.
Your brain cannot properly predict the amount of movement required with acceleration on (Google "muscle memory").
If you move the mouse 1cm on the mat and it normally moves 1cm on the screen, if that ratio changes depending on the speed you move the mouse, your brain gets confused and cannot "learn" because it is getting different results when it thinks it is making the same movement (The real world does not work like that).
This also applies somewhat to "Sniper mode" DPI shifts, but less so because it is a constant sensitivity.
Turn your sensitivity down as low as is practical. Low sensitivity = finer granularity of aim, plus the more movement you have to make, the larger tolerance there is for a margin of error.
If you use an app such as my RollMouse, you can make your mouse behave like a trackball and thus can afford to have the sensitivity very low, whilst still being able to twist your torso through the full range at max speed by flicking the mouse and picking it up mid-motion. The view keeps moving in that direction until you put the mouse back on the mat. This does not interfere with your muscle memory for aiming.
Try and ensure that you use a roughly consistent sensitivity (eg width of your mouse mat = 180 deg turn) in all FPS games -
If you play one game with one sensitivity, and MWO with another, that will not help your brain "learn" how to judge the amount it needs to move the mouse to achieve the desired aim correction.

Whilst in game, open chat and hit one of the mouse buttons. If the key appears in the chat box, but it does not trigger the in-game function that it is bound to when not in the chat box, then the problem is this:
Keys must be held for 50ms in order for MWO to properly register them - your mouse is likely pressing the key, then releasing it immediately. Many games do not like this. MWO's chat box works differently, and does not require keys to be held for 50ms.
