Its based on match score (Note: match score, not damage), and winning is the most important criteria:
Here are the match score ranges and what happens to your PSR
On Loss:
100 <= large drop
101-250 = medium drop
251-400 = even
401 >= small rise
On Win:
100 <= even
101-250 = small rise
251-400 = medium rise
401>= large rise
New accounts get a boost on the PSR change values during the cadet bonus phase, which is how experienced players can take a new account up several tiers very quickly.
