While PGI is a small studio, others are too yet get better results so part of it has to be method / approach to design.
Consider:
HBS is small but manage to produce a successful BT game even though more strategy than tactical shooter. Part of this was due to raising funds from Kickstarter so no exclusive deal needed while PGI swapped to Epic only offer for about a year to get funds.
HBS did a Beta only offering Instant Action, no Campaign or Career but allowed for good, serious bug fixes before release while MW5 seemed to have no Beta I recall and even after fixes implemented recently still has serious issues a Beta would have gotten fixed.
Just a couple of examples here.
