Apparently the item database was glitching out due to too many new item id so they reduced it by a few factors by combining them. This is a rather involved process and can take a while to implement. Give them another week or so but no later than the dec patch and probably sooner. As far as mistakes this is a fairly common one and easy enough to fix.

In reference to the title you chose : "Never touch/change a running system" is not applicable looking at MW:O and its journey over the years . Stuff changes/gets changed/gets added, in consequence things may break . Not saying breakage is inevitable, but the smallest of changes/additions can have the weirdest of consequences .

We used to have the Test Server, but we do not have it anymore.

I don't think test server would have helped anything with current issue as they didn't do anything major that could break the game... it was something they didn't see coming at all.

Right, a test server does not necessarily imply that the database infrastructure is also replicated. 'Test server' can be more or less anything. Anyone can manage a database, and even put one together without too much fuss with a little programming knowledge, but it takes a whole other level of competence to do it properly, or to be able to consolidate records / perform other restructuring tasks. It's absolutely not something you just rush into with some 'iterative' approach. I presume they tested the fix on a backup, but it's still no guarantee that there won't be any issues.

What they likely looked at QA side is the loads after the changes, the default loads then the additional queries, and hopefully they dropped into mechs that were maxed out on decals, bolt-ons, cockpit items and colors.
