It happened in the Heir to the Dragon novel around the time shortly after the War of 3039 where Theodore pushed the DCMS to its limits during the counterattack after being named Kanrei. It was before Theodore had any reconciliation with his father, and his subordinate took Theodore's extreme displeasure at his father's meddling as a sign that he should have Takashi killed (in typical Yakuza/Mafia fashion, you don't usually say to someone "boy I'd love to see that guy killed," they instead talk about how much easier life would be without person X).
Oh, and the suicide/killed thing has to do with the ritual of harakiri, where the individual mortally wounds himself, but then an assistant kills him, typically by cutting off his head. Piotr cut off his own finger in front of the Kanrei along with delivering the oyabun's head to Theodore. Yikes.
