There are some bugs in the game. Several of them disconnect you but don't show you as Disconnected. My evidence of this is having it happen to me while online with several friends. My mech was running in a straight line until it hit terrain and just sat there. My name didn't show up as disconnected in the list, but my client wasn't open and had crashed and there was no process from MWO running in the Task Manager.
I have another friend who occasionally gets the blackscreen bug. Sometimes it will clear up shortly after the map starts(he is always last to load from our group) and sometimes it won't. He isn't disconnected but isn't actually in the match. He looks like an AFKer. Yet another one has had a graphical bug where his screen went Yellow, just bright yellow. I guided him to the base and had him stand on it while the rest of us pushed forward some. He basically stood there.

The best way is still to have someone report them. Right now, with the current bugs, I report friendly fire and team killers. Not the, "Oops" friendly fire, but the kind you have a very strong feeling is intentional. Then again I think some people need to learn that Alt-Tab not only gets you out of the screen, but can get you back in while Left Mouse clicking on the screen fires those weapons. Ehem....
