Because everybody simultaneously has the best strategy in their head.

Sometimes it just can't be helped. You can ask to co-ordinate, give a general plan, do any number of things, and often people will do them - but once in awhile, everyone goes their seperate ways anyways. You could try joining a group, but if you're time-limited and committed to playing matches on your own, the best you can do is to suggest plans (edit: or follow the ones given - which is more my personal method), and stay close to allies that you can maintain speed with and who aren't suiciding.

I never see that anymore. Now people just look for the nearest ECM mech (preferably an Atlas) and stay with it in a big blob. Even the scouts :D

Even if you do get a decent plan, there's bound to be one ***** who will charge over the ridge on their own. They'll learn.

It's okay to break off on your own... the point is that

when you find the big group of enemies nestled over there

you

don't just keep trudging through them like they're not going to hit you or something.

What you should do is type, hey guys, there's 6 of them over here and 3 of them are ECM... I noticed we have no ECM and a couple A-1 Catapults... Can we just powerdown and call it good?
