It seems to happen a lot more with the latest patch, specifically with the Raven from what I've experienced, yet I haven't tested rigorously and might be misled by my point of view. Speed for testing purpose is 148 kph. Average (self) ping is 30. Following things are happening:

Group of hit boxes desynchronize from the 3D model of the mech. Issue 1: hit boxes are in reality behind "you". You can experience that by looking a mech clearly shooting behind you, then hear the shot and then see the hit on your paperdoll / loose component / die. Issue 2: hit boxes are in reality in front of "you". You can experience that by looking a mech shooting in front of you and seeing damage on your paperdoll. I have experienced issues as a light (fast) mech and against a light (fast) mech.

Hit boxes seem to overlap each others in weird way. Situation: the mech is being damaged on a part by a shot when other parts are in the way. For example, damaging (with Gauss Rifle) the rear center torso of the mech while the mech is looking towards you, and when facing side. Also, hit boxes on some mechs seem to be completely not in place with the 3D model. Leg hit boxes on Cicada and Raven especially.

This has been going on for months and the developers know. Tends to affect smaller 130kph+ mechs and is partially due to the net code that they are in the process of fixing.

My point is that it seems to be getting worst recently and on particular 3D mech model.
