-no hud,
-stuck map indicator,
-zoomed out map so much that the two caps are close to each other in the same quad
-torso twist indicator stuck, flipped or dynamically flipping map,
-missing map,
-rainbow map often with huge FPS hit, 15 or 7 fps; sometimes the fps hit is non present, shift+f10 brings fpss back to 60
-no hud + 7/15 fps bug (rainbow map is usually under it, disabling hud fixes FPSs back to 60, again shift+f10 fixes fps issues)
-screwed icons with "WWWWWW" everywhere
-screwed weapons list that keeps growing occupying the whole hud
-missing weapons with unassigned information (this gets fixed after shooting the weapons once, then the correct icon appears back into the hud)
-missing friendly radar indicators, when allied mechs are behind or under or over terrain from your position
All the above appears roughly in 3/5 of the matches I play. Regardless of the mech and the map involved and the game mode involved (although i play 95% assault and no lights at all) and usually right after the start up animation.

Bugs always occur at the start of the match, during loading. After starting sequence and when UI loads/shows you instantly know whether the bug is here. UI bugs never happen later.
I never have issues for the first match played, I think. The chance of getting some kind of bug is higher for consequent matches, at some point I thought it is always for the second match played, but it is not true. My guess would be the improper resources management (Captain Obvious reporting).
I think loading into Training Grounds have much higher chances of getting UI bug.
Bugs distribution for me:
1) "No map/minimap" is the most common - 70%
2) "Direction does not change in minimap" - 20%
3) "No hud/no IFF" - 6%, happened only 2-3 times
4) CTD happend only twice for the latest patch, had 0 crashes since Dec'12 before that. CTD never happen immediately, it is more like you start playing, move for 20-60 seconds and then lock up and CTD.
*) 'WWWWWW' or numbers for mech name(s) happens from time to time arbitrary, I almost pay no attantion to it now.

Nvidia 460GTX / ATI 5970 (both cards used, same exact issues)
Most bugs encountered frequently from the above list. (Never had spectator display when starting a round.)
Two additional Bugs:
First, IFF disappearing for some friendly players when out of line of sight. (Looking away or when behind obstructions)
Most noticable when players join mid startup sequence. Does not appear to be specific to any map.
Second, The resource colors in conquest have reversed colors for enemy and friendly capped resource bases. Red is friendly, Blue is enemy. Indicates this on both the minimap and the resource bar. However, the number indicators display color and resources collected correctly. Happens too infrequently to get any pattern.

max settings, most of the display bugs have occurred in current mechs I drive (mostly stalker but also catapult/jenner) on most maps.
If a bug occurs, it is during startup. have not had a single instance where a bug occurred after start of a match, or corrected itself during a match.
Prior to the last couple matches did sometimes get HUD issues listed above, but very rarely. <1% of matches. Now its about 20-30%.

Also as above, a few matches the last person on my lance list only displays if I have visual contact, and only that player. when asked they had no HUD issues that match.

1. For me all the HUD and minimaps and CTDs really only appeared with any sort of frequency SINCE the HM and its patch were introduced (the black screen), prior to that the game was about 90% stable and I don't recall having any HUD bugs/CTDs - if there was it was rarely (less than 5%).
2. The minimap and HUD bug seem to be random.
3. The CTD however for me starts to manifest more frequently after long MWO gaming sessions. Just as the mech is booting up it hard crashes to desktop, no logs, nothing.
4. Since updating the video driver to v320, the full HUD missing bug does not seem to manifest so far.
5. Most common bug for me is the missing minimap(and big map), complete failure of torso indcator, no IFF, and next ones that are not so common is the red crosshairs bug, reticle arm bug,

My experience is identical, it appears to be completely random and irrelevant of whether I drop in a group or alone, as well as whether I was in the Mechbay between matches or not.
Experienced in the current build, both alone and in literally all possible combinations including all of them simultaneously.
No IFF
"locked" hud (Reticles do not move, arm fire and missile locking however is still very much independent o fhte center reticle)
No targeting boxes. usually in combination with no IFF, but not always
Compass lock (top compass remains stationarry regardless of orientation)
No top Base dsplay on conquest, only total resources.
Minimap "locked" (Orientation of legs and torso always straight up, arrow turns normally)
There may be one or 2 that I`m forgetting, but I think others will have that covered.
IFF + targeting bugs definitely persist when spectating (i.E. his may be working but I can`t tell what he`s got targeted until the info comes up, ans still see no arrow or box), haven`t actively paid attention to whether other HUD bugs do.
I cannot say for sure that I have seen the rainbow minimap or a bugged overwiew map since the
I perceive the overall frequency of game starts with one or more bugs active to be about 35-40%, in another 5% or so one eventually gets introduced, but that doesn`t seem to happen too often (or my brain subliminally plays it off as "immersion" because I just took a big hit and it`s to be expected for something to break.)
Also, I cannot say that I really had bugs with any notable frequency before the patch that nerfed missiles / introduced the HM. Maybe there`s a loose end in that patch somewhere that made this more global....

PS: if it is of any relevance, I used the repair tool once on Friday IIRC, experienced no apparent improvement in stability but a drop from 25 fps to 10-15, and now am back to using SweetFX and a custom user.cfg.

The CTD has never happened in the same match as the HUD bugs, but it will usually happen to me about once an hour or so.

Where/When it appears - mostly random, I can go several games where I have no issues or it is the first issue on my first game.
Certain actions in menus - Not really. I run Full Windowed and experience a small amount of issues after I alt-tab back into the game. No rhyme or reason though.
Sequences - Once I get one bug, the severity increases. Restarting game client doesn't help.
Other issues at same time - Usually it's one bug at a time. Once I get a bug though, the chance for crashes increases.

Edit: Forgot to mention that the bugs I experienced before I cleaned my shader cache showed up maybe once every 30-50 matches per bug, ie they were very far spread apart, and only after periods of play longer then 60-90 minutes, so it might have to do with extremely high memory consumption corrupting the hud.

3. Random.
4. I alt tab a lot. Even if I don't alt tab I'll still get a bugged HUD. Seems completely random.
5. One at a time. Don't think I've noticed 2 HUD bugs at the same time. Maybe some WWWWWW text where my mech name is suppose to be with another bug. (like missing map)
Overclocked or not, I get most of the HUD bugs randomly, I'd say around 1 in 5 games. Can be the first game I play of the day or the 10th, etc. I have not noticed a pattern or something that I do on my end.

Shading: Very High
Shadows: High

Have been flipping AA on and off latley not seeing a frame drop.
I get constanant 59.9 ish FPS while runing dual screens, I alt tab often to mess with websites on the other screen manage music etc etc.
There is a HUGE frame rate drop if the full UI is missing to about 24 fps. The fps only get that low if the full UI is missing, as in no chat, map, retticle, anything... The ultimate hard core mode.
There is an occasional light drop in FPS during the more minor bugs of the missing or locked map, both seems to drag my fps down to aprox 40's.
I have no proof or much luck replicating the no UI (I dont really try to) but it seems to me that if I am havily interfacing with chat before and during the start up sequence (pre star loby to systems online) the bug seems to appear more often.
During long periods of play the bug seems to be happening more often, Could be a memory leak? Stack overflow? not releasing memory somewhere?
Had a new bug for the first time yesterday of losing my entire UI with no frame rate drop if my mech throtle was set to zero. So When I hit 'x' to come to a complete stop on a cap point my UI flickers once and completely vanishes. as soon as I barely tap 'w' the UI comes back. The mech did not need to actually be in motion, just had to have a little indication that was not '0%' thrust.
Is there a way we can submit dump files to you guys? Perhaps a built in collection system would net more data?

Several matches last night:
1. No Minimap (just a blank dark grey with the FOV and Leg arrow, that did not move.
1b. No Minimap + No IFF - same as above, but with the added fun of having no IFF indicators - no red/blue triangles either on minimap, or on my HUD for mechs in my field of vision - ie. no way to tell friend from foe.
1c. No Minimap + No Base Cap meters - so no way to tell who is winning or which bases on conquest are or are not ours.
1d. No Minimap + No IFF + No Base Cap meters. "Flying blind," truly.
2. 8-bit Hypercolor Minimap (looks like some old ASCII art overlayed on mini-map screen, and prevents use of "Board" - if I hit "B" for the map, the map is all hypercolor ASCII art.)
3. Minimap FOV and Leg indicator do not move. I can see the arrow indicating my mech facing move, but the arrow and FOV stay pointed "up" and do not move with my mech.
4. Reversed Conquest Colors - shows "Enemy Base Cap" when Friendlies are base capping, bases my team captures are in red, bases other team caps are in blue... Opposite day! Neat! *Groan*

Drop to desktop (~2% of drops)
Not sure if this was a bug, but when I enable heat or night vision after the changes they would randomly turn off and I would have to double tap N or H to re-enable them...

They always occur at the start of the match once the HUD finally loads, I've never had it occur mid match. If I CTD it also seems to occur within the first few minutes of a match.

Issue always appear from the start of a game. Occurs at random - sometimes first match of the day, sometimes no glitch until the 10th match, etc.
No action triggers it as it appears right from the start.
No pattern to its appearance. Got it twice in a row before, but most of the time at random.
No other issue noted at the same time. Glitches only appear individually, ie no IFF, no minimap, but not both at the same time.

Everything High, AA very high.
One thing I sometimes get did not see on list is if I Alt-Tab out at start of match (to kill a video I may have been watchng while waiting for match start), is I get the MOUSE CURSOR on screen which reacts to moving the reticle. Has not actual effect, just annoying as it is moving around the screen.

Only menu use is Mechlab and Pilot Lab, I looked at the options menus just now to double check settings and that's my first time in weeks.
The issue does not appear to (wait for it) appear in sequences.
No significant actions performed. I do alt-tab out while waiting for launch at times, however issues have occurred without alt-tabbing out for several matches.
Other issues: A single instance of a bug where I was repeatedly forced out of social tab/Mechlab/Pilot Lab back to the home screen. This was fixed by exiting and relaunching. Also one crash to desktop per 20-30 matches, once occurring the drop immediately after a HUD bug (specifically, no minimap) .
Sunday night I had a very few instances of minimap and large view map coordinates not matching/showing massive grids, as well as one instance of my mech's turning/twisting on the minimap being reversed; well over 50% of games with at least one disconnect, in addition to multiple mechs found wandering aimlessly, clearly unpiloted, with users still showing as connected. I mention this last only because I have only seen this occurring on this night.
On the off chance that this helps, last patch was absolutely horrible for me in regard to bugs. Since the last patch, I have experienced very, very few bugs or crashes.
