it is an online game, there are hackers out there that somehow think it is fun to cheat. There are hackers in all online games, some games do better at responding to fix/remove hack/hackers.
I have seen lately on a few occasions where a mech that I have been shooting in the back with my firestarter or jenner, and is taking fire from a fellow team mate- well they get down to yellow/orange internals on CT and magically their armor is restored on the CT. I had a T-bolt in red internals from the back in one match on HGP, and suddenly all his back armor was returned to full health. I dont worry about them much, those types of players are the kind that get reported, and also tend to be the kind that get bored with the game easily.
I would say, if you see this stuff happening over a few nights, or notice the same names in matches it occurs, to contact support. You can report players if you see the same names all the time when this occurs. If you dont, you and your friend may have some tech issue that maybe support can help you with. if nothing else, if you contact them within a good timeframe of a match that this occurred, pretty sure they can go back and search by your playername and review the match and watch for nonsense things.
In the end, dont just write it off as "must be bad ping" if it keeps happening.

My wife and i used to play MWO together and sometimes we'd spectate the same mech after we both died and our screens would often show different damaged areas and even different ammo counts.

All in all I'd say it's worth the tradeoff, given how much HSR has improved overall gameplay. I'd be a little happier if HSR could be made to reconcile (or at least attempt to do so) the view of both attacker and target, which might be more tenable now that server-authoritative effects are live, but I'd rather have HSR as it works now than go back to the bad old days where nobody could hit anything without lag shooting or luck (or unholy sacrifices to the daemons of the interweb).

And I checked to make sure there was no ammo in that particular torso, and there wasn't.

I have had also some other weird instant deaths lately. Though, I have been mastering my commandos so can't really be sure.
