Mine is when turning a Corner thinking the enemy isn't there when they actually are and getting a healthy breakfast of Vitamins B, and L.

Also, getting Domination on Grim Plexus usually promises a bad game experience for me.

- AFK/mostly lights on blue team/Myst Lynx's
- Seeing blue team scatter to the winds looking for Intel
Any game:
- Having no scouting/recon and turning blind around a corner into a firing line, especially when your mech is slow.
- Turning to face a push or positioning to trade from high ground/good position only to see your team has fled/ NASCAR'ed away to flank/ambush?? the enemy from behind cover somewhere far far away...
- Pushing when called only to discover that the person calling is a long way behind you and no one else is really keen.
- Forgetting to equip Radar Derp on a new assault/slow mech and dropping on Polar.
- Blue team leaves x assault/slow mechs behind
Mining map:
- your Alpha and Beta lances have NASCAR'd the one of the flanks, opposite Charlie lance, who are now trying to catch up... and team red pushes past the middle as a group.
Canyons/Terra maps - conquest:
- Charlie lance heads to Epsilon/( or insert cap name) by itself... and meets 2 - 3 lances of team red...
Terra Therma:
- Blue team gathers around an entrance way or two allowing a couple of the mechs, to get in one another way whilst, trying to trade versus most of team red who are all in, having good lines of fire.
Domination
- Being first to the green circle when piloting an assault or slow mech...
- Alpine peaks & domination on that side.
- Grim Plexus; when your team wants to lrm/poke/wait/hide/whatever from behind the hill outside of the circle... occasionally feeding smaller mechs to green monster... till team red pushes, unopposed around one of the sides...
The are the ones that spring to mind... bound to be more

So yeah, losing power in a fight is pretty much worse case scenario for me.

I'm curious why you accelerate and stop with your mouse? I never heard of anyone doing that. How many buttons does your mouse have? How do you fire?
