I also had a perms-fall on the new map (mining colony), didn't see my radar before I died, but think it was NEAR the middle of the map. Was trying to JJ from a low flat area up to a small ledge with \ \ \ \ orange stripes like that. Died when I got stuck on the angled striped part.

Here is the coordinates and the position where I got stuck on CrimsonStraight. I am sorry that I didn't get more screenshots of exact location, but I did not think of that at the time. If I ever get stuck there in the
same location I will be sure to add more screenshots of exact location.

Something is amiss in Tourmaline Desert. Playing the game at between 20:05 and 20:15 hours PDT on the 26th, I was trying to slide down a wall in my Kintaro 19, and suddenly I had no forward momentum and could not turn. One of my guys tried to free me up, but he wound up destroying the remainder of my weapons, and took out one of his own legs in the process. We still won the fight, but it was harsh being stuck there.
Coordinates: 4649.5 3230.0 199.5
I don’t know whether or not those will help, but I know I got stuck there. I was unable to reproduce it, but didn’t actually have an opportunity to do so during the remainder of the evening.

I was chasing a Raven, made a sharp turn, and found myself immobile.
Enemies firing missiles at me had them land on the platform behind me (to
the positioning of the legs in the image above)

Got stuck while piloting a locust underground. Was told it looked like I
was falling. Could not turn and changing speed made no difference. Once killed I fell through the world.

Mech gets stuck after jumping onto that position (cannot walk off or even jump off; jump jets will not recharge after being used.

Observed while playing tonight. This was a screenshot from inside the
problem mech (Slaphead Mk V) on Alpine Peaks near Sigma capture point.
The mech was stuck sort of bouncing up and down in midair. Turning,
engine and jump jets did not free him.
A teammate was able to walk underneath and jump jet to free him,
managing to get stuck himself... then Slaphead returned the favor...
eventually freeing them both. Unfortunately, while they were stuck,
they weren't capping Sigma and we eventually lost by three points (bummer).

River City, Skirmish, RVN-4X, stuck after JJing at the top of the citadel at coords 2177.0/1141.6/75.8
River City, Assault, GRF-1S, stuck after JJing at coords 2080.3/1207.0/29.8; was blamed with a TK after a friendly unsuccessfully tried to get me unstuck.

Piloting a Kitfox, I jumped on one of the housing units just outside the cave exit and got stuck in place, constantly falling. Coordinates 1833.7, 2562.6, 229.3
